DHHS Service Request Process 

1. File an application: Download and complete the Application for Services (PDF) from Deaf and Hard of Hearing Services.
2. Sign the Student Services Agreement: Review and sign the main agreement covering scheduling, attendance, and provider expectations.

a. Sign additional service agreements: Complete extra forms each semester for specific approved accommodations like interpreting, note-taking, or assistive listening devices.
b. Submit your Audiology Report/Audiogram: Provide a copy (not the original) of your recent audiology report to Deaf and Hard of Hearing Services.
